The following is a summary of the OASIS-based outcomes measure descriptions. See the CMS Manual for the complete measure, including details on the numerator and denominator, exclusions, and whether the measure is risk adjusted.

Percentage of home health quality episodes during which patient(s)

? Improved or stayed the same in ability to groom self

? Improved in ability to dress upper body

? Improved in ability to dress lower body

? Improved at bathing self

? Improved or stayed the same in the ability to bathe

? Improved in ability to get to and from and on and off the toilet

? Improved or stayed the same in ability to get to and from and on and off the toilet

? Improved or stayed the same in ability to manage toileting hygiene

? Improved in ability to get in and out of bed

? Improved or stayed the same in ability to get in and out of bed

? Improved in ability to ambulate

? Improved in ability to take their medicines correctly (by mouth)

? Improved or stayed the same in ability to take their medicines correctly (by mouth)

? Became less short of breath or dyspneic

? Frequency of pain when moving around improved

? Demonstrates an improvement in the condition of surgical wounds

? With Stage 2–4 pressure ulcers present at discharge that are new or worsened since the beginning of the quality episode

? Bowel control improves

? Are confused less often

? Needed urgent, unplanned medical care from a hospital emergency department, immediately followed by hospital admission

? Ended with the patient being admitted to the hospital
